If you are not a patient registered with the Clinic and require rosacea support or would like to pose a general question, you may complete the preliminary rosacea support form instead.

Please ensure you complete all fields accurately and in full as we must be able to positively associate you with your treatment file in order to prepare a reply.

Your "registered e-mail address" is that which you have submitted directly to the Clinic at the time of consultation, or online when placing an order to replenish your skin care products.
